Child's Foreskin Stuck Behind the Glans: Paraphimosis Is an Emergency

A swollen, painful foreskin trapped behind the glans should not wait for a routine clinic visit.

What is paraphimosis?

The foreskin has been pulled behind the glans and a tight ring prevents it from moving forward again. Swelling makes the constriction worse and delay can compromise blood flow.

Typical signs

  • Rapid swelling and pain of the glans and foreskin
  • A tight band behind the glans
  • Dark, blue or purple color change
  • Sometimes difficulty urinating
Emergency: a trapped foreskin that cannot be replaced needs urgent medical treatment.

What to do while seeking care

Do not repeatedly pull, use sharp objects or attempt to cut the skin. Do not delay because of embarrassment. Early reduction of swelling and replacement can prevent more serious ischemic injury.

Follow-up after reduction

The cause should be reviewed, such as forced retraction, failure to replace the foreskin after cleaning or catheterization, or an underlying tight ring. Later treatment may include observation, cream or surgery depending on the child.
